All Parcel Insurance claims are handled by Universal Parcel Insurance Coverage (U-PIC). If you have any claims questions please contact U-PIC directly at EndiciaClaims@U-PIC.com or call 800-955-4623 option 4. Claims may be faxed to U-PIC at 818-332-5092 or mailed to U-PIC, 5703 Corsa Avenue - 2nd Floor, Thousand Oaks, CA 91362.

Claims Forms Instructions

1. Fill out claim form completely, attach all paperwork and mail within 90 days from date of shipment.
2. For LOST parcels you must wait 21 days for domestic and 45 days for International. Lost claims for shipments sent to Italy require a 60 day wait before filing. Damage claims can be filed immediately.
   
3. Staple to this form the following pieces of information:

   A. A copy of the original invoice to the consignee or EBay end of auction as proof of value.
   B. A signed letter from the consignee stating the following:

   1. Date the package was mailed.
      
   2. Invoice or auction number.
      
   3. Description of shipment, (i.e. 4 Pier 1 wine glasses).
      
   4. If loss, state parcel never received, if damage, state description of damage and if repairable.
      
   5. Signature and date.
      

4. Freight charges are only recoverable if the freight value is declared in the insured value. If claim is for damage, you are required to hold onto the damaged property until the claim has reached a resolution. Failure to do so will result in denial of the claim.
